• All pages of your bank statements for the past 2 months
• Self-Employed, Corporate Tax Returns for the past 2 years (all pages)
• All pages of your retirement account statements for the last quarter
• Proof of any supplemental income
  Legible copy of your social security card
• Legible copy of your driver’s license
• Credit card account information (monthly payment balance)
